Support with Transitions

At Personalised Care Services, we understand that people often need support during significant life transitions. Whether it’s returning home from a hospital stay, transitioning from school to adulthood/services, moving to a new home, re-entering the workforce, or starting something new and unfamiliar, we are here to provide one-to-one support for a smooth transition. We will help you voice your wishes, dreams, and aspirations, acting as your advocate every step of the way until you feel confident and independent.

Transition from School to Adult Services

Here’s a brief description of how we support the transition from school to adult services:

  • Needs Assessment: We will complete a comprehensive Needs Assessment when it is identified that you need support with your transition to adult services.
  • Multi-Disciplinary Approach: If parents request respite or support with other services, the assessment will be conducted in a multi-disciplinary manner, ensuring all relevant services can contribute and prepare together.
  • Service Visits: You and your parents can visit the services you are looking to use in the future.
  • Inclusion in Reviews: We will request that our assessment worker is invited to any children’s service review.
  • Pre-Admission Assessment: If parents wish to pursue our service, a pre-admission assessment will be completed directly with you. This assessment will gather specific details about your support needs, such as managing risks, behavior, strategies, level of staffing required for specific tasks (e.g., bathing/showering), equipment used, task duration, medication needs, etc.
  • Support Planning: From the pre-admission assessment, we will inform you and your parents/Social Worker if we can meet your needs and outline the support required.
  • Referrals: Any necessary referrals following the outcome of the pre-admission assessment will be made, for example, Speech and Language Therapy (SALT), Occupational Therapy (OT), behavioral support, etc.
  • Telecare: Any telecare required to support you will be identified and sourced.
  • Staff Training: Any additional training our staff require to support you will be identified and provided.
  • Regular Meetings: Our transitions worker will regularly meet with you and your family.
  • Information Transfer: Our transitions worker, along with the key worker from your current service, will work together to establish and maintain frequent contact and begin the transfer of information.

We are dedicated to ensuring a smooth and supportive transition process, empowering you to move confidently into the next stage of your life.
